Assam Chief Minister Free Diagnostic Services Programme

Introduction

• Chief Minister Free Diagnostic Services Programme is the health care programme of Assam Government for the people of Assam.
• It was launched on 11th of May 2017.
• The main objective behind launching Chief Minister Free Diagnostic Services Programme is to provide free of cost Quality Diagnostic Services to people of Assam.
• Department of Health & Family Welfare of Assam Government is the nodal department of this scheme.
• Under Assam Chief Minister Free Diagnostic Services Programme, Government of Assam will provide below mentioned diagnostic services free of cost :-
  • Free of Cost CT Scan Services.
  • Free of Cost X-Ray Services.
  • Free of Cost Laboratory Services such as Blood Test, Imaging Scans etc.
• No Amount will be charged from any person irrespective of their income status.
• Every person of Assam whether APL or BPL is eligible to get free of cost diagnostic services under Chief Minister Free Diagnostic Services Programme.
• More than 56 numbers of laboratory tests are available under this scheme.
• Services under Chief Minister Free Diagnostic Services Programme is provided by HLL Lifecare Limited which is a Government of India Enterprise.
• Till date more than 64 Lakh Patients availed the service of free of cost diagnostic under this scheme.
• To avail the benefit of free of cost diagnostic services, beneficiary has to visit any Government of Hospital or Authorized Diagnostic Center with any of the following identity proof :-
  • Domicile of Assam.
  • Voter ID Card.
  • Aadhar Card.
  • Driving License.

Benefits

• The following benefits will be provided to eligible beneficiaries under Assam Government's Chief Minister Free Diagnostic Services Programme :-
  • Free of Cost CT Scan Services.
  • Free of Cost X-Ray Services.
  • Free of Cost Laboratory Services such as Blood Test, Imaging Scans etc.

Eligibility Conditions

• Beneficiary Should be a Permanent Resident of Assam.
• Beneficiary of Every Age Group are Eligible.

Documents Required

• Any One Proof of Residence :-
  • Domicile of Assam.
  • Voter ID Card.
  • Aadhar Card.
  • Driving License.
• Medical Prescription from Doctor in which Diagnostic Test is Prescribed.

How to Apply

• Beneficiaries has no need to apply anywhere to avail the benefit of Assam Chief Minister Free Diagnostic Services Programme.
• Beneficiary can directly visit the below mentioned places to avail free of cost X-Ray, CT Scan and Laboratory Services, wherever available :-
  • District Hospital.
  • Government Aided Hospitals.
  • Government Empanelled Hospitals.
  • Public Health Centers.
  • Community Health Centers.
  • Authorized Diagnostic Centers.
• Fill the application form of service to be avail and attach all the required documents with it.
• After initial verification, free of cost diagnostic test will be conducted for which patient does not have to pay any fees.
