Phc centere nurse dose not full fill my form last 2 months. I gave all documents in May 2024, their was 2 months already finished but they do not register my form.

Last 2 month pass away but the phc center in karamsad Nurse jagruti and ashaworker both not given satisfied answer and they do not co - opertavie. They are give me wrong details nd not register my form last to months. I was called many time they don’t gave me correct answer then I decided i call 104 nd complaint yesterday. I was 7 months pregnant mother nd i have also high blood pressure in 4 months pregnancy when i gave my all zerox document jagruti nurse but they do not work properly and sincerely. So I complained it and help me to full fill my form in namo shree yojana.
