Mode of Applying

Introduction

  • Arogya Nidhi Scheme is the major health care scheme of Assam Government.
  • It was launched in the year 2013.
  • The main objective behind launching Assam Arogya Nidhi Scheme is to provided financial assistance to the people who are facing financial crunch while dealing with exceptional rare diseases.
  • Department of Health & Family Welfare of Assam Government is the nodal department of this scheme.
  • Financial Assistance up to Rs. 3,00,000/- will be provided to beneficiaries under Assam Arogya Nidhi Scheme.
  • Financial Assistance under this scheme will be provided in the form of reimbursement.
  • Serious Diseases which requires surgery or implant or any other critical disease are eligible under Arogya Nidhi Scheme.
  • Treatment of Serious Accident Cases will also cover in this scheme.
  • Eligible Beneficiaries are eligible for reimbursement of treatment cost if the treatment is taken from Government Run/ Aided/ Empanelled Hospital or Reputed Private Hospitals.
  • The categories that are eligible for specialized treatment under Assam Arogya Nidhi Scheme are as follows :-
    • Life Threatening Diseases like :-
      • Heart Disease.
      • Heart Surgery.
      • Cancer.
      • Kidney and Urinary Disease.
      • Orthopedic.
      • Thalassemia.
      • Bone Marrow Transplant.
      • AIDS.
      • Chronic Mental Illness.
    • Injuries Caused by Natural and Manmade Disaster like :-
      • Industrial Accident.
      • Farm Accident.
      • Road Accident.
      • Rail Accident.
      • Bomb Blast etc.
  • The Annual Family Income of patient should not be more than Rs. 5,00,000/- per year.
  • There is no minimum or maximum age limit under Assam Arogya Nidhi Scheme.
  • If there is a Government Employee or Pensioner in a family then they are not eligible for Arogya Nidhi Scheme.
  • Eligible Beneficiaries can apply to avail the benefit of Assam Arogya Nidhi Scheme by filling the Offline Application Form provided below :-

Eligibility Conditions

  • Beneficiary Should be a Permanent Resident of Assam.
  • The Annual Family Income of Beneficiary Should not be more than Rs. 5,00,000/- per year.
  • Beneficiary of Every Age Group are Eligible.
  • Beneficiary Should take Treatment in below mentioned Hospitals only :-
    • Government Hospitals.
    • Government Medical Colleges.
    • Government Aided Medical Institution.
    • Government Empanelled Hospitals.
    • Reputed Private Hospitals.
  • There Should be No Government Employee or Pensioner in Beneficiary Family.

Documents Required

  • Patient Photograph.
  • Family Income Certificate.
  • Any One Identity Proof :-
    • Voter ID Card.
    • Aadhar Card.
    • Driving License.
  • Vouchers, Bills, Receipts in Original.
  • Copy of Doctors Prescription.
  • Discharge Slip.
  • Bank Account Details.
  • Copy of below mentioned card, if applicable :-

How to Apply

  • The only to way to avail the benefit of Assam Arogya Nidhi Scheme is by filling Offline Application Form.
  • The Offline Application Forms can be downloaded from here :-
  • Fill the Application Form of Arogya Nidhi Scheme carefully and attach all the required documents with it.
  • Submit the Application Form along with all the documents in any of the below mentioned offices :-
    • Directly to Assam National Health Mission Office.
    • Joint Director, District Health Services.
  • Expert Committee of Doctors of Guwahati Medical College scrutinized the received application forms.
  • Committee recommend the eligible cases fit for financial assistance.
  • Once list of eligible cases confirmed, the financial assistance under Assam Arogya Nidhi Scheme will be transferred in the bank account of beneficiary.

I had a brain tumor surgery in Apollo,chennai on 8 January 2020 which required amount between 5 to 6 lacks.As my father is death,so the financial condition become too weak and required Money is collected by selling home land.
After that on December 2022, I applied to Assam Arogya Nidhi scheme to refund the possible amount by which I can continue my higher studies.
But still I haven't got any information about the application.Its my kind request to the Assam Government to help me for continuing my life journey.
I will be thankful to you.

hospitals are not cooperative. they did not admit in arogya nidhi scheme nor in ayushman

My father have been diagnosed with serious heart problem and doctor told that he is supposed to use oxygen mask any time but we are in a condition that we can't afford oxygen cylinder please we request you kindly provide us the required amount for all his medication. More than 70%of his heart has been damaged and diagnosed by severe heart attack
